They've been laying the groundwork for Dean's little freakout nicely, waiting several episodes before he overtly states his worry about Sam. I loved Bobby's oh-so-convincing response. "No, I'm sure Sam is perfectly fine," looks away, "He's fine." And Dean's equally convincing echoing of the 'fine' sentiment. Yeah, boys. You trust Sam about as far as you can toss him, right about now. And with Ruby the Demon leading Sam down the Primrose Path, there are reasons not to trust him, but not the reason you think.
liptonrm put it much more elegantly than I can, but Sam is still himself. It's the circumstances that have changed, not the person. Yeah, he's sometimes a lot colder than he was before, but that's because he's realized that sometimes the things he considers distasteful have to be done anyway and if he hesitates or holds back (like he did with Jake the first time) the consequences will be that much worse (like dying and Dean making unspeakably bad deals with Demons to bring him back). So now he just does what has to be done and then feels guilty about it later.
And that's what is going to happen with Ruby, as well, I think. He'll do what she wants him to do, in order to save Dean, but then when all is said and done, there's gonna be a hell of a mess to clean up, and I think Sam is going to find himself in a place he doesn't want to be. I can't wait to see what happens. *grins*
